At the end Joey asks "Are you crying" to someone next to her and the voice answers "It really hit me" (or something like that). The scene zooms out and it's Pacey. Dawson gets a call from Spielberg.

When the African doctor is called in to do the clandestine operations, he doesn't operate the Turkish girl but extracts the hotel manager's kidney and gives it to the buyer. He then shares the money with the Turkish girl and the black prostitute and then goes back to his country to see his daughter. He will go back to meet up with the Turkish girl who in the mean time escaped to the US.
